How to get NSO in SM malls



Want a hassle free registration to get your birth certificate ?
You can get it on Nearest SM Department store ! How ? Here is where i get mine

Due to my working hours i just ask my friend if she can fill up the form for me .

1. Ask if there is a goverment services or customer services in getting NSO . But the sure branches are SM North and Megamall

2. Go to the line for filling NSO . Then they will give you this form


3. Fill up and submit it to the counter . (Note: The process may take 1 week)

4. There is a procesaing fee so prepare P140

5. Please keep your receipt . This is required in claiming your certificate , also bring a valid id with you :)
